Treats and provides health care services to patients under direction and responsibility of physician. A Physician Assistant is a Registered! Nurse who has received specialized training (often at a graduate level) in diagnosing and treating illnesses and providing health care maintenance. The nursing home/skilled nursing facility based Advanced Provider works in conjunction with physicians in skilled nursing facilities to ensure patients are appropriately managed in the sub-acute and/or long-term care setting. The Practitioner will provide direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Include the following; however other duties may be assigned:
Evaluates patient, performs comprehensive physical examination, and gathers patient medical data, including health history and results of physical examination.
Responsible for the treatment of members in the nursing home and skilled nursing facility settings.
Performs comprehens! ive SNF assessment to include psychosocial factors (when appli! cable) identifying the anticipated short-term and long-term needs of the member.
Identifies and refers members into appropriate disease and case management programs.
Identifies the need for hospice care as appropriate in the âterminal periodâ of chronic diseases.
Refers appropriate members to case managers for hospice program election or to be designated as a âPotential Hospiceâ candidate and monitored for decline.
This provider collaborates with the physicians for all patient related responsibilities and deals directly with skilled nursing facility staff members to coordinate patient care requirements.
Other duties as assigned.

EDUCATION AND EXPERIENCE, ! SKILLS, CERTIFICATES, LICENSES, AND/OR REGISTRATIONS:
Education: Graduation from a physician assistant educational program, accredited by the Accreditation Review Committee on Education for the Physician Assistant (ARCPA) AND successful completion of the National Commission of Physician Assistants (NCCPA) examination; must maintain status as a certified physician assistant, including completion of all requirements for continuing medical education (CME) and re-certification.
Experience and Skills: Minimum of 1 year clinical experience in acute care hospitals and or discharge planning. Familiarity with computer and ability to use Word.
Certificates/Licenses/and/or Registrations, etc..: Physician Assistant certification in the state that the will practice
Other Requirements: Excellent verbal and written communication and customer service skills. Demonstrates autonomy, organization, assertiveness, flexibility and c! ooperation in performing job responsibilities. Reliable transportation, current driverâs License, and current auto insurance. TB test, new hire and State requirements for position.
